1. A Surety Bond of Rs. 2.50 lakh (Two Lakh and Fifty Thousand) on the stamp paper of Rs. 100/- must be submitted by the candidates before joining of training.
2. After successful completion of CCHN training, a Surety Bond of Rs. 2.50 lakh (Two Lakh and Fifty Thousand) on the stamp paper of Rs. 100/- must be submitted by the candidates at the time of joining as CHO to serve for minimum three years at Sub Health Centre level HWC in UP.
3. Candidates will have maximum one additional attempt to pass the course in upcoming Session.

The candidate should be registered with Indian Nursing Council.

सामुदायिक स्वास्थ्य अधिकारी पद हेतु कुल 5000 रिक्तियाँ उपलब्ध हैं।

सामुदायिक स्वास्थ्य अधिकारी पद के लिए वेतनमान Maximum Rs. 35,500/- per month निर्धारित किया गया है।

3. Read Instructions then Proceed to the online application.
4. Fill All the Mandatory Details viz. Name, Father’s Name, Date of Birth, e-mail Address, Mobile Number, and Qualification, etc in the Application form.
5. Follow the instructions and complete the registration process step-by-step for getting a Registration Number & Password.
6. Upload the Scanned Copy of Documents in the Prescribed Size.
7. Before submitting, the Application Form Must Check the Preview and All Column Carefully.
8. Take Printout of your Application form for Future Reference.
9. Online Application Can be Submitted on or Before 09 August 2022.
